The Health Assessment Advisory Service at Crewe Assessment Centre is a ground floor assessment facility. It has three assessment rooms.
Parking is available opposite the Assessment Centre (CW1 2JZ) and to the side of the building, off Delamere Street. The Assessment Centre entrance is at the side of the building on the left, next to the car park entrance barrier.

- PIP Assessment for people claiming Personal Independence Payment (PIP)
- Work Capability Assessment for people claiming Universal Credit (UC)
- Work Capability Assessment for people claiming Employment and Support Allowance (ESA)
- Other benefit assessments for people claiming a range of benefits such as Industrial Injuries Disablement Benefit (IIDB), Disability Living Allowance (DLA) for children or Veterans UK

Directions
Train
Crewe train station is approximately 1.1 miles from the Assessment Centre. Turn left out of the station onto Nantwich Road. Turn right onto Edleston Road, which becomes Market Street. Turn left onto Market Square and pass the bus station on your right, before reaching Wellington House on your right.
Car
From the A532:
Turn right onto Hightown, and then onto Flag Lane. Turn left onto St Mary’s Street, and left again onto Delamere Street. The Assessment Centre will be on your left.
From the A534:
Turn left onto Edleston Road, and then follow the road onto Market Street. Turn left onto Chester Street, then right onto Lawrence Street. Turn left again onto Delamere Street. The Assessment Centre will be on your right.
